About the role: Chosen Care Group is currently recruiting Home Care Workers with a minimum of six months experience in delivering Personal Care and housekeeping tasks. You need level 1 in Health and Social Care or equivalent, effective communication skills, and be prepared to work flexibly to meet our Patients' needs in the community. We are passionate about making people's lives better, and we treat our patients and communities as our families since we value their concerns.
Main duties:
- Assist patients with personal care tasks, such as washing, dressing, preparing meals, feeding and toileting.
- Perform light housekeeping tasks, such as dusting, vacuuming, and changing bed clothes.
- Assist in the safe lifting, transferring, repositioning and movement of patients.
- Support all aspects of medication and accompany patients to medical appointments.
- Observe, monitor, and record patients' physical and emotional well-being and promptly report any changes to senior staff.
- Encourage patients to participate in social and recreational activities while helping them maintain independence.
- Ensure effective administration and reporting is carried out for every patient you provide care and be able to work under minimal supervision.
