About the role: Chosen Care Group recruits a qualified Nurse with at least two years of experience treating patients with diseases, disorders, or injuries. You should hold a nursing qualification and be in the process of registering with or already registered with the Nursing and Midwifery Council (NMC) with an active PIN. Effective communication skills are essential, and you should be prepared to work flexibly to meet our patients' needs in the community. We are passionate about improving people's lives and treat our patients like our families, as we value their concerns. All our patients are based in their homes or extra care facilities; your workplace will be their home. You will visit multiple patients, and the patients may vary from week to week.
Main duties:
- Delivering and managing a patient’s healthcare plan in the patient's home according to the GP’s instructions.
- Checking clinical readings of the patients, record and report where needed.
- Administering and maintaining a record of all prescribed medication.
- Check and assess the patient for infection, bedsores, and wounds.
- Educating, advising, and training health care assistants, senior care workers and caregivers in the household.
- Dressing or redressing wounds and assessing the progress of healing.
- Developing and improving the healthcare plan in collaboration with other healthcare personnel in the household.
- Providing palliative care as needed to keep the patient comfortable.
- Make recommendations for devices and tools to improve the patient’s quality of life.
