About the role:
Chosen Care Group is currently recruiting Care Manager with a minimum of one year experience in domiciliary / extra care service. You need to have level 5 in Health and Social Care or equivalent, and proven understanding of CQC regulations, inspections and what it means to achieve a 'Good' or 'Outstanding' rating.
- You should be able to draft comprehensive care plans and risk assessments.

Main duties:
- Manage the day-to-day operations of the care service, ensuring that the highest standards of care are provided to all patients.
- Monitor to evaluate the quality of care provided and ensure that all patients' needs are met promptly and efficiently.
- Carry out regular assessments of patients to identify any changes in their needs and develop appropriate care plans and update regularly.
- Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient.
- Audit to maintain accurate records of all patients, care plans and related documentation.
- Liaise with family, health care professionals and social workers to ensure the best possible care for each patient.
- Ensure that all relevant regulations and policies are always adhered to.
- Organise and attend special trainings along with the staff delivered by professionals in hospitals or other settings in the community.
