We are seeking a compassionate and dedicated Health Care Assistant to join our team. The ideal candidate will provide essential assistance to individuals in need of care, ensuring their comfort, safety, and well-being. This role is pivotal in enhancing the quality of life for those we support, whether in a residential setting, nursing home, or through home care services.
Responsibilities
- Provide patient care and support to individuals in various settings, including senior care and assisted living facilities.
- Assist with daily living activities such as bathing, dressing, and meal preparation.
- Administer medication as prescribed and monitor health conditions.
- Perform heavy lifting when necessary to assist clients with mobility or transfers.
- Maintain a clean and safe environment for clients by adhering to health and safety regulations.
- Collaborate with healthcare professionals to ensure comprehensive care plans are followed.
- Document care provided and report any changes in client condition to the appropriate personnel.
Requirements
Service user at home with his family and requires a 24-hour care package to manage his health and daily living needs effectively. Diagnosed with Motor Neuron Disease (MND) in February 2024. Has complex care needs due to his condition, impacting mobility, communication, and respiratory functions. Requires continuous ventilation via a tracheostomy. He experiences breathing problems and has a history of ventilator-associated pneumonia. Has limited verbal communication due to tracheostomy and disease progression. Utilizes a communication board and electronic whiteboard for interaction. Can interpret and communicate using facial expressions and gestures. Dropping calls staff will be assisting Tier 3 staff with transfers and incontinent care.
