Choices Healthcare is an established health and social care organisation that supplies a variety of health and social care staff to the NHS, various local authorities, and private clients alike. In addition, we provide supported living accommodations and both internal and external healthcare training. We currently operate across Southeast England with scope for expansion.
We are looking for an experienced individual who is committed to delivering person centred care.
The RoleThe Care Coordinator will assist with all day-to-day operations of the care services we provide, ensuring the smooth running of care services that we deliver. You will be required to lead a team by example, working as part of a team to maintain the personal care of clients, respecting the dignity of the individual and promoting independence. You will ensure that care services are consistently provided to a high-quality standard. Responsibilities include:
- Organising your day-to-day requirements
- Ensuring administration and recording of medication has been completed
- Liaising with health professionals and recording to the clients' computerised care plans
- Assisting with the induction, training and supervision of support staff
- Experience of supervising a care team
- Ability to show empathy/understanding and experience of the needs of our clients
- Good leadership skills with the ability to communicate effectively
- An excellent telephone manner
- Excellent written and verbal communication skills
- Highly proactive, self-motivated, and enthusiastic
- Previous Care Coordinator/Carer/Admin experience would be an advantage
- Computer literate including with the use of Excel, Word, and Emailing
- Driving licence is essential
- NVQ is preferable; however, it is not essential
