Our client is recruiting for two additional Wellbeing Matters Live Well Community Connectors.
Location: Brunswick House, M6 and other locations across Salford
Hours: Full-time - 35 hours per week (Monday to Friday, with flexibility required)
Contract Type: Initial 12 Month fixed term contract
Salary: £30,328 per annum (NJC scale pt19 pro-rated to 35 hours)
Pension: 5% employer contribution
Annual Leave: 28 days plus statutory holidays

Job Overview: An exciting opportunity has arisen for 2 x Wellbeing Matters Live Well Community Connectors to join the organisation, one as a Champion for neurodiversity and the other as a Champion for low level mental health. Working directly with individuals, the Community Connector will act as a bridge between the person, their families or carers, and the rich network of community resources available across Salford. Their work will focus on reducing social isolation, fostering belonging, promoting independence and empowering people to improve their wellbeing through meaningful community connections.
Main Duties of the Job:
- Build positive working relationships, accepting referrals from the Adult Social Care multi-disciplinary team in this city-wide role, and maintaining open communication to ensure individuals receive timely and appropriate support.
- Develop strong communication with the people you support, ensuring their strengths, goals, and personal preferences shape the support offered and that they are connected to relevant opportunities within their community.
- Provide clear, up to date information and maintain strong local knowledge, sharing details about community initiatives, resources, activities, and support offers across Salford.
- Facilitate meaningful connections, helping individuals meet others, engage with community groups, feel confident in social settings, and build supportive peer networks across Salford.
- Offer holistic, person centred and practical support, including attending groups with individuals where helpful, supporting them to navigate services, overcome daily challenges, and access financial, social, creative, health, or wellbeing related activities that meet their needs.
- Promote inclusion and advocate for accessible community environments, sharing knowledge and working with partners such as VCSE organisations, development workers, and community venues to reduce barriers and ensure individuals feel welcome and able to participate fully.
- Promote learning, volunteering, and employability potential, connecting individuals to opportunities through Elevate, WorkWell, the Growth Company, and wider Salford offers, recognising the wellbeing benefits of purposeful activity.
- Manage a caseload effectively, building and maintaining strong working relationships with voluntary, community, and social enterprise organisations to ensure smooth onward referrals and coordinated support.
- Work as part of a wider team, liaising closely with the Wellbeing Matters neighbourhood Community Connectors and Volunteering Development Workers, ensuring individuals can access community provision and working under the direction of the Social Prescribing Operations Manager.
- Raise awareness of specialist support, providing insight and information to the wider Wellbeing Matters team relating to your specialism (Neurodiversity, Older Adults, or Low Level Mental Health).
- Support integrated working across the system, collaborating with key city partners such as health services, adult social care teams, mental health teams, carers’ networks, and VCSE organisations to ensure individuals receive a coordinated and responsive service.
Champion for Neurodiversity Responsibilities:
- To support neurodivergent individuals and their families by helping them navigate community resources, access inclusive opportunities, and build a sense of belonging.
- Focus on reducing social isolation and ensuring neurodivergent people can engage in community life in ways that feel meaningful and supportive.
- Offer a safe space to discuss goals and aspirations, signposting individuals to relevant activities and services, and facilitate connections with peers who share lived experience.
- Support individuals to explore learning, volunteering, and employment pathways that reflect their strengths and interests, helping to remove barriers and promote confidence in work related settings.
Champion for Low Level Mental Health Responsibilities:
- To provide a safe, non-judgmental environment for individuals to explore their goals, strengths, challenges, and coping strategies.
- Through supportive conversations, the Connector will help individuals build confidence, take positive steps toward managing their own wellbeing, and overcome barriers to accessing timely, community-based support.
- To connect people to creative, physical, social, and community led activities that uplift mood, reduce isolation, and encourage independence.
- The Connector brings lived or professional experience in supporting individuals with low level mental health concerns, and demonstrates empathy, active listening, and a solution focused approach.
