Full -Time, Monday through Friday, 9:00 am - 5:00 pm

  • Works alongside clinicians and community providers(when applicable) to share insights and promote coordinated, person-centered care.
  • Helps reduce isolationby connecting people to groups, resources, and community activities.

What You'll Bring

Education & Experience

  • High School Diploma or Associate’s Degree with at least 1 year of experience in a human services‑related field
  • Equivalent combinations of education and experience may be considered

Required Clearances & Qualifications

  • PA Criminal Clearance
  • PA Child Abuse Clearance (if applicable)
  • Valid Driver’s License
  • Not listed on any Medicaid/Medicare Exclusion lists
