Overview

CHSW Liaison Nurse (RSCN) – Cornwall Foundation Trust (Secondment/Temporary 18 months)

Location – Cornwall, with travel within the region. Salary: £47,849 – £54,097 per annum, pro-rata if part time, plus enhancements for night, weekend and bank holiday working, and mileage payments. Secondment/Temporary (18 months). 32 – 37 hours per week (dependent on preference). Usual working hours will be Mon to Fri, with some shifts including weekends and night/bank holidays to cover end of life care as required.

Join CHSW, an established charity delivering hospice care for children with life-limiting conditions and their families in the South West of England, with bases at Little Bridge House (North Devon), Charlton Farm (North Somerset) and Little Harbour (Cornwall). This role sits within a partnership between Children’s Community Nursing Team (Cornwall Foundation Trust) and CHSW to enhance palliative care for children and young people in Cornwall and CHSW.

Responsibilities

  • Have a CHSW caseload of children living in Cornwall (with flexibility) to provide more local accessible support.
  • Ensure hospice and local care plans are up to date and aligned.
  • Undertake pre-hospice, stay assessments/preparation ensuring care plans are up to date.
  • Undertake reviews/assessments as required.
  • Provide family listening support or signposting to listening support, including bereaved parents.
  • Visit children who are hospitalised in Cornwall for advice, liaison, and support.
  • Work collaboratively with local teams attending clinics, improving communication and liaison.
  • Participate with and support any end-of-life (EOL) care at home (roster) for CHSW children being provided locally, providing leadership and clinical care ensuring 24/7 cover as required.
  • Link with the hospice bases when Cornwall children visit, maintaining clinical skills through occasional shifts.
  • Attend meetings and training ensuring connection with both hospice and community teams.
  • Liaise and work closely with the team of Clinical Psychologists supporting access to psychological care.

Qualifications and Experience

Registered Children’s Nurse (RSCN) with experience in caring for children with life-limiting conditions, complex needs or chronic illnesses. A senior nursing role that enables development of skills and enhances palliative care support for children and young people within Cornwall and CHSW.

Requirements and Expectations

  • Senior nursing role with leadership and clinical duties in palliative care.
  • Collaboration with Band 7 palliative care nurses and other professionals to lead, develop and promote palliative care and strengthen links between the hospice and local teams.
  • Travel across Cornwall to hospital bases, children’s homes, schools and other environments as required.
  • Visibility across Somerset and within the CHSW hospice.
