Core Responsibilities:
- Deliver direct services—including assessment, planning, respite, mentor services, and referrals—to program participants.
- Provide comprehensive services consistent with the program plan.
- Offer supportive services, including transportation when needed.
- Attend community events with program participants.
- Work under direct guidance and supervision of an assigned staff member.
- Document all services per program guidelines and complete monthly/quarterly reporting or billing.
Candidate Qualifications:
- High School Diploma/equivalent required; some college preferred.
- Minimum one year of relevant experience.
- Age 21 or older.
- Availability for evenings and occasional weekends.
- Effective oral and written communication skills.
- Well organized, detail oriented, resourceful, self‑directed.
- Valid driver’s license, auto insurance, reliable transportation required.
Compensation & Benefits:
- Hourly rate: £16.60–£18.00; salary commensurate with experience; bilingual candidates may receive additional compensation.

Location & Schedule:
- Services are offered in 9 surrounding Northern counties (Stephenson, JoDavies, Boone, Winnebago, Lee, Ogle, Dekalb, Whiteside, Carroll) to families in their homes, community locations, or virtually.
- Staff may work from the Regional Rockford Office or a remote location.
- Position is Reduced Full‑time (32 hours per week) with flexible scheduling, including evenings and weekends.
