An exciting opportunity has arisen for a registered nurse to join our community care team in SMASH. This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home. Travel is an essential part of this role. Please note you must have a full UK driving licence, use of a car and NMC registration to apply for this post.
Main duties:
- Work under the direction of the Community Nurse Team Leader and be responsible for the assessment and management of health needs of an identified population and for the development, implementation and evaluation of programmes of care within a range of target groups including end of life care and patients with long term conditions and complex nursing needs.
- Support patients to remain in their own place of care where possible, maximise independence and quality of life; hold a designated caseload and deputise in the absence of the District Nurse.
- Liaise with healthcare professionals and other agencies to ensure effective service delivery.
- Work within the agreed framework for Community Nursing Services; assess, provide and review holistic nursing care, advice and information to patients and carers in home or appropriate care settings using evidence-based knowledge.
- Undertake initial assessments on receipt of specific referrals and develop, implement and evaluate care plans that meet physical, social, emotional and environmental needs.
- Provide skilled nursing care including wound care, leg ulcer management, administration of medication via syringe pump, care of intravenous lines, venepuncture and catheter care.
- Record all patient contacts accurately on EMIS and Trust documentation; maintain confidentiality in line with NMC guidelines.
- Promote clinical excellence and evidence-based care; maintain competencies within the Trust’s framework.
- Participate in End of Life (EOL) care delivery and health education to promote self-care and appropriate planned and opportunistic care.
- Manage discharge planning and ensure equipment and aids meet patient needs and are maintained; provide safe use guidance as required.
- Refer to other practitioners when needs exceed own scope of practice or require longer-term support; prioritise and manage own caseload; participate in quality monitoring and governance activities.
- If applicable, contribute to Non-medical Prescribing in line with local policies and formulary.
