Medicines Optimisation Pharmacy Technician in Wakefield

Job Summary

37.5 hours per week – fixed term for nine months.

This role will be based at Fieldhead Hospital in Wakefield with occasional cross cover at our other Pharmacy bases.

Standard working pattern is 9 am until 5 pm with weekend commitment.

The post is full‑time, 5 days per week.

Flexible working can be discussed on appointment.

Responsibilities

• Coordinate all aspects of patients’ medicines‑related journey, working with speciality pharmacists and the multidisciplinary team to ensure safe, seamless and integrated transfer of care.
• Support clinical teams with supply, storage and management of medicines within inpatient areas.
• Complete medicines history and reconciliation on admission.
• Provide information to patients and carers about their medicines and support adherence and understanding.
• Plan and support timely leave and discharge, utilising the near‑patient dispensing process to ensure medicines are received in a timely manner where appropriate.
• Support transfer of care into other settings and work closely with the primary‑care team to support users on discharge.
• Accuracy‑check medication for leave and discharge.
• Support the training and development of trainees.
• Participate in weekend and bank holiday rota.
• Maintain flu vaccination up‑to‑date to protect staff and patients.

Person Specification – Qualifications

• NVQ Level 3, BTec or similar nationally recognised qualification in pharmaceutical sciences.
• UK professional registration as a Pharmacy Technician.
• NVQ A1 assessor qualification (desirable).

Experience

• Demonstrable experience as a pharmacy technician (essential).
• Accuracy checking (desirable).
• Hospital pharmacy experience (desirable).
• Setting up ward‑based pharmacy services (desirable).
• Managing files and stock balances on Pharmacy Stock Control system (desirable).
• Problem‑solving and investigation of stock discrepancies (desirable).
• Provision of medicines management to new services linked with community pharmacies (desirable).
• Discharge planning (desirable).
• Line management experience (desirable).
• Self‑administration of medicines (desirable).
• Training
• Nationally recognised accuracy‑checking qualification or ability to demonstrate competency at this level (essential).
• Accredited checking technician status (essential).
• Medicines management training (desirable).
• Team‑leading training (desirable).
• System management training on Pharmacy Stock Control programme (desirable).
