30 hours per week. An exciting opportunity to be part of the Integrated Community Stroke Rehabilitation Team in Barnsley. The role is part of a multi‑disciplinary team of nursing and allied health professionals in both community and inpatient settings, carrying out designated duties in a competent manner. It requires self‑initiative, good communication and effective time management. Travel by vehicle is essential for the post. All employees are strongly encouraged to keep their flu vaccinations up to date to protect staff and patients. At the time of advertising, this role does not meet the minimum requirements set by UK Visas and Immigration for sponsorship. We are, however, happy to accept applications from candidates who can prove their right to work in the UK or via alternative visa routes.
Key Responsibilities
- Work as part of a multi‑professional team in inpatient and community settings to deliver safe and compassionate care to patients.
- Carry out rehabilitation programmes set by therapists and nursing staff, supporting patients in their recovery.
- Assist clinical staff with observations and recordkeeping of patient physical observations in line with the observation policy.
- Provide personal care, including moving and handling of service users with equipment such as hoists and wheelchairs.
- Respond to verbal or physical aggression, using de‑escalation techniques and restraint or seclusion as appropriate.
- Offer support, encouragement and communication to patients, families and carers during the rehabilitation journey.
- Participate in training, education and clinical supervision to develop skills and reflect on practice.
- Work independently and as part of a team, with flexibility to attend a 24‑hour/7‑day roster where required.
- Ensure compliance with all policies, procedures, protocols and guidelines, and maintain accurate care planning and activity records.
- Travel by vehicle in connection with the duties of the post.
