Job Summary
An exciting opportunity has arisen for a well‑motivated, enthusiastic staff nurse to work for South West Yorkshire Partnership NHS Trust within the cardiac and pulmonary rehabilitation service. This community‑based team provides quality care and support for patients undertaking either the cardiac or pulmonary rehabilitation programmes. The service delivers specialist care to patients in their own home and in community venues around Barnsley. The successful candidate will assess and treat patients with a wide variety of cardiac and respiratory conditions.

Key Responsibilities
Assess and treat patients with cardiac and pulmonary conditions, including heart attack, cardiac surgery, and COPD.
Triage referrals, conduct initial assessments, monitor patients during exercise, deliver education sessions and record electronic patient notes.
Participate in the delivery of a cardiac/pulmonary rehabilitation programme with the multidisciplinary team, supervising exercise in a gym setting.
Plan and deliver packages of care, liaise with other health professionals, statutory and voluntary agencies to ensure a coordinated approach.
Maintain accurate patient records and statistical returns within required timeframes.
Advise patients on health promotion to achieve local and national health targets.
Act as a mentor for students and participate in training activities.
Engage in regular clinical supervision and maintain competency through training and continuing professional development.
Qualifications and Experience
Essential: Registered Adult Nurse; Evidence of meeting PREP requirements.
Desirable: Evidence of cardiac/pulmonary diploma or equivalent.
Personal and Professional Attributes
Ability to be proactive, work both independently and as part of a team.
Strong verbal and written communication skills; good time‑management and reliability.
Capacity to travel across the Trust as required; ownership of a vehicle for travel purposes.
Knowledge of government legislation and documentation affecting NHS and primary care.
Demonstrated health promotion skills and good understanding of cardiac and pulmonary conditions.
