We are looking for an experienced Health Care Support Worker to join the Kirklees Learning Disabilities Community Health Team. The team is multidisciplinary and provides specialist health support to both individuals and their wider support networks across the Kirklees locality. It offers a range of opportunities to develop skills working alongside nursing, occupational therapy, speech & language therapy, physiotherapy, psychology and psychiatry. Your role will involve working under the supervision of a registered nurse. You will deliver specific activities and planned programmes of care for a designated group of service users.
In addition to personal care duties such as bathing, toileting, dressing and feeding, you will operate hand‑held equipment, assist with the use of hoists and wheelchairs and respond to incidents of verbal or physical aggression using de‑escalation techniques where appropriate.
Responsibilities- Provide safe, compassionate planned care that maximises recovery and rehabilitation potential.
- Maintain therapeutic relationships with service users, staff and families, using a polite, positive and empathetic approach.
- Assess ongoing care needs, identify problems and adjust care plans within your competence.
- Collect and record observations (e.g. blood pressure, blood glucose, urine analysis) and support the clinical care team.
- Ensure all support adheres to relevant policies, procedures and national guidelines.
- Work flexible, structured rosters and contribute to a 24‑hour on‑call service where required.
- Show initiative, judgement and honesty in considering alternative approaches and escalating concerns to the nursing team.
- Promote a respectful and dignity‑respecting environment for service users and colleagues.
- GCSE (or equivalent) in English and Maths (grade 3‑9).
- Level 2 qualification (or equivalent) or ability to demonstrate Level 3 competencies.
- Care Certificate (or required to obtain within 12 weeks of commencement).
- Providing personal care and related procedures for individuals with health and social care needs.
- Working in a caring role at Band 2 or equivalent competence.
- Experience of dealing with the public and working effectively as part of a multidisciplinary team.
- Strong decision‑making, prioritisation, user and carer involvement, and safe handling of violence and aggression.
- Excellent communication, administrative and IT skills, and an understanding of confidentiality and Trust values.
- Resilience and empathy in emotionally demanding and hazardous situations.
- Will attend mandatory Trust training and any further refresher courses as required.
- Complete training in the prevention & management of violence & aggression, moving & handling, and related health & safety procedures.
