Job Summary

This is an exciting opportunity to join South West Yorkshire Partnership NHS Foundation Trust as a Clinical Pharmacist supporting mental health services across Kirklees. You will have real influence, professional freedom and the chance to shape how pharmacy supports mental health care. The role is 22.5 hours per week, part‑time and flexible, permanently based in Priestley Unit and Folly Hall Mills.

Main Duties and Responsibilities

  • Deliver specialist pharmacy services to inpatients at Enfield Down, Lyndhurst and the Priestley Unit, and support Kirklees community mental health teams (intensive home‑based treatment teams and well‑being and treatment teams).
  • Provide direct patient care, patient consultations, medication counselling, complex medication reviews and rationalise and optimise medicines for mental health conditions and concurrent physical conditions.
  • Prescribe where appropriate and qualified.
  • Support departmental and trust‑wide governance agenda, contribute to audit and service development projects.
  • Supervise and train junior staff and provide clinical supervision.
  • Participate in the Saturday‑morning and out‑of‑hours on‑call rota (approximately one week every three months).
  • Play a key role in trust‑wide medicines optimisation initiatives and in delivering the new Medicines Optimisation and Pharmacy Strategy.

Key Result Areas

  • Take drug histories on admission.
  • Review medication to ensure safe and effective use.
  • Formulate and update individual care plans.
  • Conduct therapeutic drug monitoring for specific drugs.
  • Educate patients about their drug therapy.
  • Develop evidence‑based treatment protocols and support implementation.
  • Facilitate compliance aids and self‑medication schemes.
  • Undertake therapeutic drug monitoring, clinical audits and practice research.
  • Record clinical interventions, complete query record forms and report on trends.
  • Promote evidence‑based cost‑effective prescribing.
  • Contribute to the dispensing of clinical trials in line with ICH GCP and European Directives.
  • Provide clinical training for pharmacy undergraduates, pre‑registration pharmacists, post‑graduate pharmacists and technicians.
  • Support nursing staff and Medicines Management pharmacy technician with the self‑administration scheme.
  • Report and monitor medicines‑related incidents or complaints to minimise future risk.
  • Maintain links with other professionals (dietitians, nurses, doctors) and recommend service changes.
  • Ensure compliance with legal requirements, Health and Safety, Data Protection, COSHH.
  • Participate in CPD, performance reviews and the development of the Medicines Management strategy.
  • Provide clinical and ward pharmacy services as required during absence or leave of other Trust principal pharmacists.
  • Undertake all other related duties as required.

Person Specification – Essential

  • Self‑motivated with well‑developed communication skills.
  • Evidence of continuing professional development in line with GPhC.
  • Team player.
  • Teaching, tutoring and mentoring skills.
  • Experience supervising and directing junior staff.
  • Computer‑literate – pharmacy systems, Microsoft Office, internet searching.
  • Bachelor/Master's degree in Pharmacy.
  • GPhC registration as a Pharmacist.
  • Completed pre‑registration training.
  • Substantial demonstrable experience post‑registration as a pharmacist.
  • Professional manner.
  • Adaptable.
  • Current driving licence and access to a car during the working day (essential, reasonable adjustments considered).

Person Specification – Desirable

  • Specialist knowledge of mental health pharmacy.
  • Skilled in audit planning and delivery.
  • Diploma in clinical pharmacy (general, community or psychiatric).
  • Membership of CMHP or RPS.
  • Experience providing care to people with mental health problems.
  • Hospital pharmacy experience.
  • Experience in Medicines Information and associated systems.
  • Provision of medicines education to users, carers and healthcare professionals.
  • Completing audit projects.
