We have an exciting opportunity for a Community Nurse in our Kirklees Learning Disability Health Team. The team is a comprehensive multi‑disciplinary group, including psychiatrists, psychologists, learning disability nurses, occupational therapists, physiotherapists, speech and language therapists, dieticians and healthcare support staff. The successful candidate will understand the complex needs of people with learning disabilities and deliver safe, effective and personalised care to meet the needs of service users. You will be committed to improving outcomes for our service users, demonstrating excellent interpersonal and communication skills, and applying health promotion to empower patients.

Key Responsibilities

  • Provide specialist nursing support for adults with learning disabilities in a variety of community settings, maintaining and improving their health and wellbeing.
  • Work holistically with service users, families, carers and support networks, ensuring a person‑centred approach to care.
  • Support and educate other health providers, making reasonable adjustments and facilitating bespoke care pathways to ensure equal access to health care for people with learning disabilities.
  • Promote the service to external providers and participate in health promotion activities.
  • Contribute to service development, governance, policies and procedures, adhering to NHS guidance and statutory requirements.
  • Manage enquiries and direct referrals in line with team operational policy and participate in the team duty rota.
  • Maintain professional registration, demonstrate commitment to statutory codes of practice and pursue continuous professional development.
  • Safely custody, transport and administer medicines, including IM preparations where appropriate.

Qualifications & Experience

  • Registered Learning Disability Nurse, or Registered Mental Health Nurse with significant learning disability experience.
  • Demonstrated ability to assess and manage people experiencing mental health problems or learning disabilities, including behavioural challenges and risk assessment.
  • Experience in integrated multi‑disciplinary environments, participating in team meetings, case conferences and professional meetings.
  • Ability to deliver therapeutic techniques in one‑to‑one and group settings.
  • Experience of mentoring or supporting learners, and supervising students or trainees where possible.

  • Ability to undertake the duties and demands of the post.
  • Excellent communication skills, both written and verbal.
  • Ability to work independently and as part of a multidisciplinary team.
  • Current driving licence and access to a car during the working day.
  • Willingness to work flexibly, including extended hours, to meet service user needs.
  • Reliable, enthusiastic, motivated and resourceful with a professional attitude.
  • Knowledge of legislation (Mental Health Act, Health & Safety, Care Programme Approach, Care Act, Mental Capacity Act, Human Rights Act, Safeguarding).
  • Understanding of Clinical Governance principles.
  • Aware of cultural diversity and ability to incorporate this into care planning.
  • Knowledge of audit processes and IT systems.

Person Specification – Desirable

  • Relevant post‑basic qualification and training.
  • Experience of mentoring/supporting learners or demonstrating effective mentoring skills.
  • Support of colleagues and supervision of students or trainees.
  • Holistic working with adults, addressing co‑existing mental and physical health needs impacting wellbeing.
