We have an exciting opportunity for a Band 6 practitioner to work in Calderdale in a SWYT role in partnership with the Lower Valley Primary Care Network (PCN). These roles are now established in Calderdale and provide specialist mental health support in a local setting in primary care.

Key Responsibilities

  • Conduct mental health assessments with all people referred to you, determining their needs and the most appropriate service or agency to meet those needs.
  • Provide brief therapeutic work with the person referred, make referrals to other services or agencies, or advise them of other sources of support.
  • Build and maintain effective links with all disciplines within the PCNs, working as part of the integrated Primary Care MDT through the emerging Integrated Neighbourhood Health system.
  • Provide easily accessed advice for integrated care teams and support identification of training needs on mental health issues. Participate in providing training and supervision for staff within the PCN as required.
  • Undertake training, consultancy and support roles within the team and the wider integrated care workforce.
  • Work autonomously, manage your caseload efficiently, and adjust the physical environment where possible to facilitate client work.
  • Maintain professional integrity, confidentiality, and comply with relevant codes of ethical and professional conduct.
  • Liaise with external agencies as required and provide advice on risk management and service coordination.
  • Engage in regular supervision in line with professional and employer policies, and supervise junior staff where required.

Qualifications & Experience

  • Essential: Registered Mental Health Nurse, Social Worker or Occupational Therapist.
  • Post‑registration experience in mental health and experience in a community setting delivering 1‑to‑1 mental health services.
  • Proven experience of brief therapeutic work with referred individuals.
  • Basic keyboard skills and strong active listening, communication, and influencing skills.
  • Current UK professional registration and a valid driving licence with access to a car during working hours.
  • Desirable: PSI training.
  • Relevant CPN qualification.
  • AMHP qualification.

Special Knowledge & Skills

  • Understanding of responsibilities under the Mental Health Act 1983, Care Programme Approach, Clinical Governance, Risk Assessment and Child Protection Legislation and NICE guidelines.
  • Ability to analyse and present complex information, prioritise workload and inspire confidence in service users, carers and staff.
  • Supervisory leadership skills.
