Job Summary

An exciting opportunity has arisen for a well‑motivated, enthusiastic Respiratory Nurse to work for South West Yorkshire Partnership NHS Trust within the BREATHE service. This community‑based team provides specialist assessment and review of patients with a confirmed respiratory diagnosis. The service is responsible for delivering specialist care to patients in their own home as well as in our community clinics and venues around Barnsley. The successful candidate will be expected to assess and treat patients with a wide variety of respiratory conditions. The team is committed to personal development, training and wellbeing.

Key Responsibilities

  • Work autonomously assessing and managing own clinical caseload supporting patients with a range of respiratory conditions in a community setting, including those requiring End of Life care and home oxygen assessment and therapy.
  • Use and apply specialised skills underpinned by advanced theoretical knowledge of professional practice to maintain professional competence and fitness to practise as a Respiratory Nurse Specialist.
  • Tri‑age, assess and manage a range of complex presentations, taking into account physical, psychological and social care needs, demonstrating evidence‑based knowledge, investigative and analytical skills and specialised assessment techniques.
  • Formulate and implement treatment plans based on current evidence and expert clinical reasoning, integrating the principles of self‑management to enable individuals to function independently and reduce inappropriate hospital admissions.
  • Enable people to receive an early and accurate diagnosis and provide relevant information and support to manage their respiratory disease.
  • Participate in multi‑disciplinary team meetings, contributing to decisions regarding patient care.
  • Work alongside Primary Care GPs and Practice Nurses, managing patients within the respective Primary Care Network and providing targeted support where rates of respiratory disease per practice population are highest.
  • Collaborate with respiratory and medical teams within secondary care to facilitate early supported discharge of patients following an emergency admission, supporting seamless care.
  • Manage referrals to the service from medical, nursing staff and other sources, ensuring patients are seen within required timescales based on clinical need and attending to urgent assessment and support.
  • Initiate, plan and implement induction, training and education of junior team members, the broader community nursing teams and students.
  • Provide appropriate supervision and caseload support for junior members of the team.
  • Understand and apply National Standards such as NICE guidelines, National Service Frameworks and British Thoracic Society guidelines.
  • Lead the development and delivery of specialist education programmes to enable best practice in the care of individuals with respiratory conditions.
  • Contribute to the development and review of policies, procedures and standards relating to the service, using an evidence‑based approach.
  • Support the clinical governance framework to maintain high standards of care.
  • Undertake nurse‑prescribing where required and in accordance with trust policy.
  • Maintain complete, accurate and up‑to‑date records in accordance with NMC guidelines and trust policy, recording all contacts and activities, and inputting data into the IT system timely.
  • Adhere to NMC scope of professional practice and trust policies, ensuring ongoing professional competency and development through mandatory training.

Communication With Others

Communicate effectively, empathetically and culturally sensitively with patients, representatives, carers and families, using interpreters where necessary. Provide clear information to members of the BREATHE service and collaborate with primary, secondary and voluntary agencies to ensure seamless patient care. Liaise effectively with GP practices and other health professionals in the region.

Qualifications

Essential

  • First level nursing qualification.
  • Graduate‑level education and/or currently pursuing a Masters level qualification.
  • Diploma or degree in Respiratory Disease or in progress.

Desirable

  • Professional teaching qualification.

Special Knowledge/Skills

Essential

  • Recent, up‑to‑date knowledge of COPD, oxygen and respiratory care, including management of acute exacerbations and acutely unwell patients.
  • Excellent communication, interpersonal and team‑working skills.
  • Ability to work autonomously and prioritise workload to ensure effective management.
  • Sound understanding of clinical governance, with experience of quality issues and audit; ability to apply principles to the workplace.
  • Effective networking and relationship building with other healthcare/multi‑agency professionals.
  • Good presentation skills.
  • Proficient in Microsoft Word, PowerPoint, Outlook and internet use.

Physical Attributes

Essential

  • Ability to undertake the duties and demands of the post and maintain a satisfactory sickness record over the previous two years.

Training

Essential

  • ARTP accredited or willing to undergo accreditation upon commencement.

Desirable

  • Nurse Independent Prescriber.

Personal Attributes

Essential

  • Ability to work evenings and weekends, with flexibility to meet service needs.
  • Reliable, motivated and enthusiastic.
  • Effective communication on all levels.
  • Mobile on a daily basis; able to work independently and as part of a team.
  • Valid driving licence and access to a car during the working day (reasonable adjustments considered for candidates who cannot drive).

Experience

Essential

  • Post‑registration nursing experience.
  • Recent experience of respiratory care and clinical examination.
  • Substantial experience working within a nursing team and/or multi‑disciplinary/multi‑agency setting.

Desirable

  • Recent experience working in the community setting.
