We are looking for an enthusiastic Band 3 Health Care Assistant to support our Neighbourhood Nursing Service (NNS) in delivering nursing care to housebound patients. The role involves wound management, chronic disease support, promotion of self-care and terminal/palliative care, working across planned and crisis pathways, and providing compassionate care across a seven-day shift pattern. Full-time and part-time hours available. Hours may include weekends and bank holidays. Applicants must be prepared to work flexibly, including 24-hour shift patterns for certain duties.
Key Responsibilities
- Contribute to care planning and provide high-standard safe and compassionate care.
- Work across planned and unplanned pathways, supporting patients to remain well at home or in long-term care settings and preventing unnecessary hospital admissions.
- Develop and maintain therapeutic relationships with patients and families.
- Attend to personal care needs, including moving and handling of service users.
- Apply knowledge of long-term health conditions to give advice and support.
- Exercise independence under supervision, using initiative and judgement to deliver tasks.
- Participate in training, education and clinical supervision to develop skills.
- Assist with verbal/physical aggression using de-escalation techniques and, where required, restraint or seclusion.
- Operate equipment such as hoists and wheelchairs as appropriate.
- Work within a 24-hour, 7-day rostering arrangement.
- Collaborate with primary, secondary, social care and voluntary sector partners.
Person Specification
Training & Development
- Willingness to undertake Trust mandatory training and job development.
- Knowledge of service-user safety, including prevention of violence and aggression, and moving and handling.
Essential Experience
- Providing personal care and procedures for people with health and social needs.
- Experience at Band 2 or equivalent level of competence.
- Working effectively in a team and independently where required.
- Demonstrated initiative, decision-making and prioritisation skills.
- Experience of working with the public and in caring environments.
- Commitment to user/carer involvement.
Essential Physical Attributes
- Satisfactory sickness record over the previous 2 years.
- Ability to meet Occupational Health requirements for the post.
Qualifications
- GCSE A-D or equivalent in Maths and English.
- Level 3 apprenticeship or equivalent (RQF/NVQ) in Health & Social Care.
- Care Certificate or ability to achieve within 12 weeks of appointment.
Desirable Qualifications
- Level 4 units in Health & Social Care.
- Experience of the speciality area.
- Willingness to pursue further study toward Level 4.
