Advanced Nurse Practitioner in Barnsley

Are you looking to progress your career? We are looking for an experienced nurse to work within our Urgent Community Response Service (UCR).

You Will:

• Work as an advanced Nurse delivering high level clinical assessments & reviews to patients in their own home, implementing a comprehensive plan of care to prevent unnecessary hospital admissions.
• Be involved in the delivery of care for patients as part of ongoing developments including UCR response standards & out of hours provision, supporting virtual ward patients.
• Work as part of a wider MDT of AHPs, specialist nurses, primary care, acute & social care colleagues.
• Monitor & improve standards of care through supervision, clinical audit, evidence-based practice, learning & development, & provision of clinical leadership.
• Be expected to work across a seven-day week shift pattern including weekends & bank holidays. The shift pattern predominantly includes days & afters, but you may be required to work occasional nights.
• Be able to travel independently between sites & to patients' homes.

Main duties of the job:

The Successful Candidate Will:

• Be an experienced nurse, have completed the Nurse Prescriber course & Advanced Physical Assessment & Consultation Skills (APACS) qualification.
• Demonstrate a broad range of specialist nursing clinical expertise supporting high quality person-centred care. This role requires resilience & ability to problem solve quickly & effectively.
• Be expected to provide & deliver cover across our neighbourhood footprint, working in an integrated way with therapy, community nursing & acute, adult social care as part of the wider system.
• Work autonomously & be able to suggest & contribute to improvements within the pathway/team.
• Contribute to the on-going development of clinical practice & standards.
• Have a key role in training & education, participate in regular supervision & appraisals contributing to clinical leadership facilitating a culture of learning/reflective practice.
• Have line management responsibility for staff with health & wellbeing a key consideration.

KEY RESULT AREAS:

The postholder will:

• Be responsible for the delivery of safe and effective nursing practice across our Neighbourhood Teams, ensuring a 2-hour response to crisis call outs, providing ongoing support as appropriate.
• Work as part of the multi-disciplinary urgent community response service to support unwell patients in domiciliary care homes and their own homes. To undertake face-to-face assessment, diagnosis and treatment for community patients presenting as unwell to prevent hospital admission, where appropriate and keep patients safe within their own homes.
• Ensure clinical systems are in place to support and monitor practice and performance and support the Clinical Team Leader in the operational delivery of the service.
• Hold accountability for all service user allocations across the Crisis 2 hour nursing pathway having shared responsibility for the clinical caseload management for the team.
• Provide trust wide leadership and support within their own discipline or urgent crisis response.

Person Specification:

Essential:

• To be willing to undertake post graduate training to further skills, knowledge, and expertise.
• Evidence of Continuing Professional Development.
• Clinical Supervision training and experience.
• Experience at senior level and ability to demonstrate development of advanced leadership and management skills.
• Experience of providing nursing services to patients in community settings.
• Experience of working with individuals with complex health needs and behaviours that challenge.
• Experience of managing and assessing risk whilst working independently.
• Experience of working in a multi-disciplinary environment.
• Evidence of a wide range of clinical skills, abilities, and experience.
• Understanding of the specific health needs of people with complex nursing needs.
• Ability to lead and motivate a team of staff.
• Excellent written and verbal communication skills and negotiation skills.

Desirable:

• Moving and Handling training.
• Positive Behaviour Management Training.
• Specialist training in areas relevant to the patient group, e.g. Long Term conditions, minor illness, minor ailments and injuries.
• Relevant qualification Community Nursing Management training.
