Overview
Activity Co-ordinator – Band 3 role at Cheswold Park Hospital. This NHS Foundation Trust role covers rehabilitation and recovery services in Calderdale, including the Community Based Rehab Team and the Community Rehabilitation Unit. The Band 3 Occupational Therapy Assistant/Activity Co-Ordinator works with an excellent therapy team within the Multi-Disciplinary Team to provide holistic therapeutic care focused on recovery.
Responsibilities
- Support registered therapy staff by assisting with the implementation of interventions after assessments and providing feedback on progress to support care evaluation.
- Undertake some initial assessments which feed into formulating care plans with individuals.
- Develop individualised activity plans around service users’ hobbies and interests and help service users connect with services/groups, supporting incorporation into their weekly routines.
- Support rehab-specific groups alongside the Multi-disciplinary Team and provide information and practical support with functional rehabilitation.
Location and Hours
Location: 72 Victoria Road, Elland, West Yorkshire HX5 0QF. Working pattern: Full-time, flexible working.
Essential Qualifications and Experience
- GCSE A-D or grade 3-9 in English and Maths or equivalent.
- Level 2 qualification, or working through, with the ability to demonstrate competencies at Level 3. Care Certificate or required to achieve within 12 weeks of commencement.
- Willingness to undertake Trust mandatory training and ongoing development.
- Experience in providing personal care and working in a caring environment at Band 2 or equivalent; ability to work effectively as part of a multidisciplinary team; good IT/keyboard skills; understanding of confidentiality; resilience to pressure.
Desirable
- Language skills other than English; experience supervising/inducting others.
- Full driving licence and access to a car during the working day (reasonable adjustments considered for any applicant unable to drive due to a disability).
