Care Coordinator - Nurse Liaison

We have an exciting opportunity for a fixed-term 12-month position as a Care Co-ordinator – Nurse Liaison.

Position Details:

• 1 x 18.75 hours per week, fixed term for 12 months.

Care co-ordinators play a vital role within a practice by proactively identifying and working with patients. The Nurse Liaison role is essential in supporting the Practice Nursing Team across Royal Primary Care. They work closely with nursing teams to manage a caseload of patients, acting as a central contact point to ensure appropriate support is provided to patients and their carers. The role involves helping patients understand and manage their conditions and addressing their changing needs.

Key Responsibilities:

• Patient Assessment: Evaluating healthcare needs and ensuring appropriate care.
• Coordination of Care: Collaborating with healthcare professionals to develop care plans and facilitate referrals.
• Education: Providing information to patients and families about medical conditions, treatments, and resources.
• Advocacy: Acting as a patient advocate to respect their needs and preferences.
• Follow-Up: Monitoring patient progress and updating care plans as needed.
