Hospital Community Pathfinder – Cheshire Community Action

Location: Based at the Countess of Chester Hospital

Hours: 37 hours per week

The Hospital Community Pathfinder is a pivotal role within integrated discharge at the Countess of Chester and Ellesmere Port Hospitals, responsible for enhancing the hospital discharge process by facilitating smooth transitions for patients from hospital care to their homes through coordination with the wider voluntary sector and other partners. This role aims to reduce unnecessary delays in hospital care and improve patient outcomes by creatively seeking alternative community-based provisions through social prescription and utilising 3rd sector knowledge.

Duration: 12-month project (with possibility of extension)

Location:Based at the Countess of Chester Hospital
