Chest Heart & Stroke Scotland is seeking a Supported Self-Management Facilitator to join our team in Dumfries & Galloway. In this hybrid role, you will help individuals affected by chest, heart, and stroke conditions access our supportive self-management programme.
The ideal candidate will have excellent communication skills and a positive attitude towards people with disabilities. Full training will be provided, and a driving licence is required as travel will be involved.
