This is a unique and exciting opportunity for any social worker wanting to join this specialist team who work closely with the Adults with Learning Disability social work teams, Transitions team, CHC team, Mental Health teams and colleagues in Commissioning. Office bases are in Ellesmere Port, Chester and Winsford and the expectation is that staff work in the office 1-2 times a week and at home or out on visits the remainder of the week.

The Complex Care and Review team is an extremely effective team consisting of a Team Manager, Practice Manager, Social Worker, Social Care Assessors and Occupational Therapy staff. The team has recently expanded to include a specialist accommodation team and an additional review team for a specific piece of work relating to our in-house provider services. This is an experienced team of highly motivated, hardworking, and skilled professionals. We are looking for candidates who demonstrate these skills and have a passion to drive forward positive outcomes for individuals regardless of their level of need or stage of life they are in. Candidates will be required to work collaboratively and creative with colleagues, and to have an in-depth knowledge of the Care Act 2014 and a sound knowledge of current legislation, policy and procedures within adult services.

The team is unique in that it completes reviews taking a ‘whole service/provider’ approach. In doing so they work closely with providers and commissioners to improve the standards of current services. This includes setting clear outcome focused care plans, taking an innovative approach regarding effective use of current services, and scrutinising services to ensure commissioned support is required and being used appropriately in keeping with a person-centred and strength-based approach. The new roles will involve reviewing our service users with learning disabilities who access our day service provision.

We are looking for dedicated and highly motivated team players to join us as Social Workers working in line with the Care Act 2014. Our Social Workers play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

You will have the opportunity to;

You will have:

  • CQSW/CSS/Diploma/Degree in Social Work or equivalent (Essential)
  • Social Work England Registration (Essential)
  • Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
  • Statutory and policy base of Social Work (Essential)
  • Appreciation of recent developments in service provision.
  • Ability to communicate effectively with customers, carers and families.
  • Ability to cope positively with change and an ability to promote positive changes to make a difference.
  • Awareness of importance of multi-disciplinary working and an integrated approach.
  • Ability to carry out assessment and support planning in line with relevant legislation.
  • You are confident, energetic and enthusiastic as well as sensitive and resilient.
  • Self-motivating individual with the passion to make a difference in adult social care.
  • Resilient personal stress management.

This is a hybrid post which means working in one of our Council offices in locations across the borough as well as the opportunity to work from home where suitable.
