Being an Assistant Reablement Coordinator means supporting people in their own homes for a short space of time to assist them to reach their full potential. This may include managing a variety of aspects of daily living skills, setting clear and achievable goals to help people rebuild their former independence and confidence to sustain independent living at home and in their communities. You will effectively work within the reablement team to deliver a high-quality reablement service with the aim of promoting and sustaining people’s independence living in the community. A driving licence is essential criteria for this community-based role.
