About the role

  • Provide guidance and practical support to care leavers aged 16–25, helping them prepare for independence.
  • Work in partnership with professionals across housing, health, education, and employment services.
  • Co‑produce Pathway Plans with young people and key individuals in their lives.
  • Maintain regular contact with care leavers and ensure access to the Cheshire East Care Leavers Local Offer.
  • Deliver duty support services alongside other Personal Advisors and keep accurate, timely records.

About you

  • You hold an NVQ Level 3 in Advice and Guidance or Health and Social Care.
  • You have excellent organisational skills, strong personal resilience and a passion for improving outcomes for young people.
  • You have experience working with diverse backgrounds and are committed to promoting positive outcomes for all care leavers.
  • You enjoy building relationships and working in partnership with other agencies.
  • You have strong interpersonal skills and thrive in a relationship‑based practice approach.
