About the role
- Provide guidance and practical support to care leavers aged 16–25, helping them prepare for independence.

- Maintain regular contact with care leavers and ensure access to the Cheshire East Care Leavers Local Offer.
- Deliver duty support services alongside other Personal Advisors and keep accurate, timely records.
About you
- You hold an NVQ Level 3 in Advice and Guidance or Health and Social Care.
- You have excellent organisational skills, strong personal resilience and a passion for improving outcomes for young people.
- You have experience working with diverse backgrounds and are committed to promoting positive outcomes for all care leavers.
- You enjoy building relationships and working in partnership with other agencies.
