An opportunity has become available to work within the Care4CE Mental Health Reablement Team. The team offer support to adults with mental health issues using the reablement model. This is a permanent position for Grade 6, 16 hours a week. The days preferred by the candidate can be discussed at interview; the hours are primarily 9am – 5pm, although some early evenings may be required. The service is office-based at Macclesfield Town Hall, but the role requires being able to visit our referrals at their homes and transport them to community resources. A mileage rate is paid for the use of a vehicle.

About the Role: The referrals to the team are from various teams (CMHT, Social workers, GP, Mental Health teams at the hospital). The team offers support with social care issues such as housing, budgeting, and social isolation. We encourage mental health wellbeing by using coping strategies and self-help booklets. About you: Experience of working in a Social Care or Health Care setting, ideally with experience of supporting referrals with mental health issues. Experience of supporting service users and carers, and developing good working relationships.
