Contract: Permanent | Various contract hours on offer from 15 to 37 hours per week
Pay: Starting at flat rate ÂŁ26,403 [pro rata, per annum], ÂŁ13.69 per hour, incremental for 5 years to ÂŁ28,142 [pro rata, per annum], ÂŁ14.59 per hour. [with yearly cost of living pay rises] [Excluding enhanced rates at weekend and bank holidays].

About the Role
In this vital role, you will help people regain independence after illness or hospital discharge. Working closely with Social Care and NHS teams you’ll work with individuals in their own homes—helping them rebuild skills for personal care and daily living, while also carrying out assessments for long‑term care needs. You’ll attend initial care visits and have opportunities to develop skills in risk assessing and equipment provision. As part of our Prevent, Reduce, Enable, rapid response service, you’ll help prevent hospital admissions by delivering same‑day support during health crises. This includes basic health monitoring (such as blood pressure and temperature checks) and low‑level healthcare tasks and rehabilitation exercises.

Core service hours: 8am–10pm (service runs 7am–11pm). Shifts comprise of mornings, afternoons and evenings which include travel time. Alternate weekends paid at time and a half. Bank holidays paid at time and a half (double pay on Christmas Day & Boxing Day). Mileage paid at 52.2p per mile (over 1000cc) or 46.9p (under 999cc/electric). Uniform, DBS, and full training provided. A clean driving licence with Business Use car insurance required.
