We are looking for enthusiastic and motivated individuals to join our adult social care teams in permanent part time social care assessor roles. There are two vacancies, one in Sandbach and another in Nantwich. You will be part of a busy frontline adult social care team.
We are looking for enthusiastic individuals who understand adult social care, mental capacity and safeguarding issues. The candidates should have excellent communication skills and the ability to develop and work both autonomously within Adult Community Services and the wider multi-disciplinary arena. You will need to have good organisational and time management skills as this is key to effectively managing caseloads.

In this role you will be expected to carry out basic needs-based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response in order to construct an individual Care Plan. Implement, co-ordinate and maintain individual Care Plans and act as Care Manager to ensure that services are arranged to meet the identified needs of vulnerable people. Negotiate and liaise with other professionals, statutory and voluntary agencies, relatives and carers about care arrangements in order to maintain the best possible service provision for clients and carers.

About you: NVQ3, or equivalent experience in Social Care. Experience of working with vulnerable people and their carers in a Social Care or Health setting. Knowledge of assessment and care management process, including how to undertake needs based assessments.
