Permanent Grade 7 Social Care Assessor, Preparing for Adulthood transition team. Full time, 37 hours.
This role is vital in supporting young people as they move from children’s services into adult social care and ensuring that their transition is planned, person‑centred and outcome focused.
The PFA Team works across the whole of Cheshire East Borough Council and offers flexible hybrid working arrangements. Staff have access to work hubs including Macclesfield Town Hall and Delamere House, with the team predominantly hot‑desking at Delamere House in Crewe.
The successful applicant will need to demonstrate a strong understanding of the Care Act 2014 and be able to complete strengths‑based, person‑centred assessments within appropriate timescales. They must also have a sound knowledge of the Mental Capacity Act 2005 and be willing to complete capacity assessments and contribute to best‑interest decisions with support if needed.
A key requirement of the role is to be able to work in a multi‑disciplinary way with Children’s Services, Education, Health partners and other professionals. The postholder must be able to communicate with and support young people from the age of seventeen, using both verbal and non‑verbal communication methods depending on individual need. They should also have some understanding of SEND policy, statutory guidance and associated frameworks and be able to apply these in practice.
The role requires the social care assessor to work consistently in line with Preparing for Adulthood outcomes, ensuring that young people are supported to develop independence, engage in their communities, access education or employment opportunities, and plan confidently for adult life. Creativity in support planning is essential, as this work involves tailoring approaches to the aspirations, strengths and needs of each young person.
About you:
- Carry out basic needs‑based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response in order to construct an individual Care Plan.
- Implement, coordinate and maintain individual Care Plans and act as Care Manager to ensure that services are arranged to meet the identified needs of vulnerable people.
- Monitor, evaluate and review individual Care Plans, identify and respond to significant changes by modifying the Care Plan to ensure that the identified needs continue to be met in the most cost‑effective way.
- Negotiate and liaise with other professionals, statutory and voluntary agencies, relatives and carers about care arrangements in order to maintain the best possible service provision for clients and carers.
- Inform clients and carers of the financial implications of care services, undertake financial assessments and encourage clients to access benefits advice in order to maximise income to clients, carers and the department.
- Prepare and maintain client records/systems, both written and electronic to ensure the provision of accurate up‑to‑date client‑related information to the group.
