About the Role

You will provide fast-paced triaging, assessments and interventions for adults who present at the front door, with physical, learning, or mental health needs that impact on their ability to manage within their home environment. You will be supported if needed to identify any potential for improvement through joint working with other community therapists, the use of equipment and/or adaptations and Assistive Technology devices. You will be required to identify the immediate needs of service users who require adaptations to their home environment and to consider their functional needs and the potential impact of the built environment and will develop solutions that will meet the client’s needs and maintain their independence and safety. About you

We are looking for:

  • Enthusiastic and well-rounded HCPC registered Occupational Therapist with knowledge of health and social care and keen to learn and develop their skills in all areas of Occupational Therapy.
  • The candidate should be self-motivated with excellent communication skills and the ability to work both autonomously and within the occupational therapy team and wider MDT.
  • Organisational skills and time management are key to efficiently managing caseloads.
  • The candidate should be able to provide a strengths-based approach to care and understand a variety of conditions and their impact on individuals.
  • It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
