Cheshire East are seeking skilled Social Workers to join our Children with Disability Service.
We are looking for:
Candidates who are proactive, with excellent communication skills and the ability to work autonomously and in partnership with health colleagues. They should have good organisational and communication skills.
The ideal candidate will have a strong background in social work, who is enthusiastic and has excellent communication skills, alongside a passion for working with children and young people with disabilities. It is important that candidates are able to work autonomously and in collaboration with other agencies to deliver effective services.
This is an exciting opportunity to make a meaningful difference in the lives of our children, their families, and caregivers. As a key member of our team, you will be responsible for completing reviews of care packages, developing creative support plans, and ensuring that the voice of the child is at the centre of all planning.
Key responsibilities include:
- Completing reviews of care packages and developing holistic support plans
- Working closely with colleagues and multi-agency partners to ensure seamless service delivery
- Liaising with partners within and outside the council to ensure efficient and effective service provision
