HEALTHCARE COORDINATOR/LPN – Midlands (Elgin) Division

REPORTS TO: Healthcare Supervisor/DON

WORK HOURS: 40 Hour Work Week

SUPERVISES: None

Qualifications

  • High school diploma and completion of approved school of nursing, with state certification by S.C. Board of Nursing.
  • Acquire and maintain certification in First Aid, CPR, and CPI (non-crisis intervention).
  • Subject to annual TB screening.
  • Valid driver's license from state of residence and ability to operate a motor vehicle for 15 passengers or less.
  • Furnish proof of liability insurance.

Physical and Functional Requirements

Physical: Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting (patients with assistance), fingering, grasping, feeling, talking, hearing, repetitive motions, communication, writing, reading, driving (including nights), counting, work double shifts as necessary, running, and smelling.

Worker is subject to both indoor and outdoor environments and may be exposed to temperature extremes of heat and cold for short time periods, noise, hazards including the possibility of physical assault, property destruction, and infections. Protective clothing is required to prevent exposure to infection.

Job Duties

  • Provides general nursing care to CHESCO Services individuals as per local, state, and federal guidelines.
  • Provides routine nursing procedures (medical appointments, vital signs, etc.) and paperwork (care plan, MAR, progress notes, TD assessments, etc.) per agency medical guidelines and in accordance with M.D.'s orders.
  • Coordinating services with outside health agencies; serves as primary liaison for all community health professionals and promotes effective working relationships with these professionals.
  • Participates in pre‑admission evaluation of individuals selected for placement in terms of nursing services; assists in the development of training programs upon admission within the health‑related areas; completes necessary appointments and paperwork for new admissions.
  • Schedules, attends and assists Coordinators with the Drug Reviews; ensures individuals receive appropriate medications as prescribed by the psychiatrist; initiates consent forms for psychotropic medications.
  • Monitors and notes monthly any discrepancies in medication administration records, medication storage, medication supplies (prescribed and OTC meds), follow‑up on MD appointments, implementation of MD recommendations, vital signs, and dietary needs; provides monthly nursing summaries to the Coordinators.
  • Provides 24-hour/5‑day a week and shared weekend and holiday on‑call medical coverage for individuals; if not within a 45‑minute response range, prior notification must be given to Supervisor.
  • Schedules all medical appointments for individuals; coordinates with Coordinator or Caregiver to provide coverage to the appointments as needed.
  • Ensures Coordinators are updated on the medical status of the individuals.
  • Responsible for the maintenance and upkeep of the medical charts; ensures that the medical charts are current.
  • Provides and documents staff/consumer/caregiver in‑services/training relating to medical/health issues.
  • Provides health care counseling to individuals as it relates to personal needs; smoking, diet, exercise, proper hygiene, etc.
  • Participates as a member of the interdisciplinary team in single‑plan meetings, medication reviews, etc., and prepares required nursing reports.
  • Completes other duties as identified or assigned.
