Chen Med expands its team and seeks a Patient Continuity and Needs Advocate (PCNA) to help eligible patients and families access Medicaid and federal resources in line with state and federal policies.

The PCNA collects documentation and initiates follow-ups as needed.

Key duties include supporting enrollment, maximizing benefits, coordinating follow‑ups, using an EMR system to document issues and responses, ensuring HIPAA-compliant records, and performing other duties as assigned by the manager.
