The LPN is a licensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centres running smoothly. Duties vary by setting, location and size. In addition to administrative duties, the LPN performs clinical duties under direct physician supervision in accordance with state medical practice acts. The LPN supports Physician-led Care Teams focused on providing excellent primary care for a specific patient population.

Essential Job Duties / Responsibilities

  • Measure and record patient vital signs; record patient interview and medical history.
  • Catalog and communicate patient needs to the PCP, clinician, specialist and/or other clinical personnel.
  • Prepare patients for examinations and perform routine screening tests; assist physician with exams; explain treatment procedures and physician instructions to patients.
  • Observe patients and report any changes in patient conditions to the PCP, clinician, specialist or other clinical personnel.
  • Perform non-critical medical and therapeutic procedures based on medical instructions; collect phlebotomy and other lab specimens; perform basic waived lab tests.
  • Prepare and administer medications; change dressings, apply bandages, remove sutures and perform other first-aid procedures; use CPR skills when necessary, all under physician supervision.
  • Maintain supplies, equipment, stocks and sterilize instruments; practice OSHA safety standards; perform accurate, legal and ethical documentation at all times.
  • Process patient phone messages, return calls and route them to other team members as appropriate; call patients to obtain and relay pertinent information for the physician.
  • Upon physician approval and authorization, call prescriptions to the pharmacy; use the ChenMed Rx system to support medication dispensing.
  • Provide health coaching to a defined group of patients to support healthy lifestyle choices; follow up with coached patients via weekly calls.
  • Perform other duties as assigned and modified at manager’s discretion.

  • Knowledge of medical products, terminology, services, standards, policies and procedures.
  • Excellent interpersonal and customer service skills with compassion and empathy toward patients and families.
  • Exceptional oral and written communication, time-management and organizational skills.
  • Ability to collaborate with team members, including physicians, nurses, case managers, front desk staff and centre managers.
  • Problem-solving mindset focused on patients and team goals.
  • Basic phone and computer operation skills.
  • Detail-oriented to ensure accuracy of reports and data.
  • Maintain effective and organized systems for timely patient flow.
  • Stay calm in busy or stressful situations.
  • Proficiency in Microsoft Office Suite products and other related software.
  • Willingness to travel locally and/or regionally up to 80% of the time to assist other centres.
  • Fluency in spoken and written English.

Education and Experience Criteria

  • High school diploma or equivalent required.
  • Graduation from a nationally accredited practical or vocational nursing school required.
  • Current, active LPN license in the state of employment required.
  • IV Therapy Certification required if mandated by the state board.
  • Minimum one year of work experience as an LPN required.
  • Minimum three years of overall clinical experience preferred.
  • Experience with geriatric patients a plus.
  • EMR system experience preferred.
  • Basic Life Support certification required within first 90 days of employment.

Pay Range $20.20 – $28.83 per hour. Final compensation depends on experience, education and location. The position may include bonuses.
