Advanced Nurse Practitioner

Job summary Cheadle Medical Practice and Alvanley Family Practice are looking for a motivated, caring and clinically curious Advanced Nurse Practitioner to join our friendly and supportive team. This role would suit an Advanced Nurse Practitioner with Primary Care experience who is an independent prescriber. We are looking for someone who is committed to high-quality, patient-centred care and who is willing to undertake a mixture of minor illness/acute style appointments as well as care home and ideally Women's Health responsibilities. The post will primarily be based at Cheadle Medical Practice, although the successful candidate must be willing and able to work across both Cheadle and Alvanley Family Practice when required to meet patient and service needs. Main duties of the job

• Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
• Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
• Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
• Read, manage and action laboratory results acting as lead clinician, processing normal and investigating abnormal values.
• Complete medical reports, insurance reports and other non NHS reports on behalf of the practice
• Lead patient clinical care in our designated nursing home providing first point of contact for staff and a weekly ward round and acute visits where necessary.

Job description Job responsibilities The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Particular emphasis will be on carrying out acute home visits, supporting frail and palliative patients and our patients in nursing homes which will require a weekly ward round. There will be a requirement for the individual to lead on our Multidisciplinary team for complex cases across the health and social care system.

Communication

• Undertake mentorship for more junior staff, assessing competence against set standards
• Undertake clinical learning and development in the area of Womens Health, as part of the continued growth of Womens Health Wednesdays, working alongside Operations Managers to enhance the service.
• Disseminate learning and information gained to other practice staff in order to share good practice and inform others about current and future developments (eg courses and conferences)
• Assess own learning needs and undertake learning as appropriate
• Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning
• Work with the Managing Partner, Operations Managers and GP Partners to develop a holistic approach to health and wellbeing, using innovative practice to link medicine, community solutions and social care to address those patients that may be lonely and isolated.
