The predicted salary is between 37900 - 44000 £ per year.

You'll support older people living with frailty by carrying out thorough assessments and tailored interventions that help them stay safe, independent and confident in their daily lives. Working as part of an experienced and supportive multidisciplinary team, you'll play a key role in delivering the community frailty pathway. You'll use your clinical skills to assess need, plan care and guide colleagues, working across the Integrated Care Centre and community settings. Your work will help improve patient experience and ensure people receive the right support at the right time.

What you’ll do:

  • Complete comprehensive assessments and deliver specialist occupational therapy interventions.
  • Work autonomously while contributing to multidisciplinary decision‑making.
  • Manage and prioritise a specialist caseload in a responsive and flexible way.
  • Support junior staff through supervision, mentoring and shared learning.
  • Contribute to service development, audits and continuous improvement.

Qualifications:

  • A registered Occupational Therapist with strong clinical skills in frailty or community care.
  • Experience working within multidisciplinary teams and managing complex caseloads.
  • Clear communication skills, with the ability to work sensitively with patients and families.
  • Confidence using clinical reasoning to plan and adapt care.
  • Commitment to ongoing learning, with support and supervision provided.
