This is a great opportunity to build skills in pulmonary rehabilitation while supporting people living with respiratory conditions to improve their health and confidence. You'll work as part of a friendly multi‑disciplinary team who deliver cardiac and pulmonary rehabilitation services across the area. Your work will help people understand and manage their condition, reduce hospital admissions and feel more in control of their health. You'll collaborate with specialist nurses, community teams and other partners to provide safe, effective and person‑centred care.
Responsibilities
- Carry out holistic assessments and create personalised rehabilitation plans
- Deliver nurse‑led care, education and support to people with respiratory conditions
- Monitor progress, review treatment plans and respond to changing needs
- Keep accurate clinical records following Nursing and Midwifery Council guidance
- Work closely with the wider team to ensure smooth, safe and coordinated care
Qualifications
- A Registered Nurse (Adult) with knowledge of respiratory or long‑term condition care
- Ability to assess needs, make sound clinical decisions and communicate clearly
- Good organisational skills and confidence working both independently and with others
- Willingness to learn, develop and take part in supervision, training and service improvement
- Basic IT skills and the ability to travel across the service area when required
