After 3 decades as a partner in this practice, I am retiring and seeking a colleague who will value the same ethos that has made this place such a rewarding home for my career. We are a financially stable, premises owning, well established practice with a list size of around 10,500 patients. We have a largely working population, with low overall deprivation, yet a healthy mix of backgrounds and needs. Our team is one of our greatest strengths: from our dedicated administration and reception staff to our skilled nursing team and nurse practitioners. Main duties of the job

• We are committed to safe, manageable, patient-centred care: 25 appointments per day, with 15-minute consultation slots

Job responsibilities

There will be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

• The delivery of highly effective medical care to the entitled population
• The provision of services commensurate with the GP contract
• Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly, referring patients to other services/agencies in a timely manner, utilising practice and any other local guidelines
• Generic prescribing adhering to local and national guidance
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
• Effective management of long-term conditions
• Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
• Process administration in a timely manner including referrals, repeat prescription requests and other associated administrative tasks
• Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways
• Maintain accurate clinical records in conjunction with good practice, policy and guidance
• Work collaboratively, accepting an equal share of the practice workload
• Adhere to best practice recommended through clinical guidelines and the audit process
• Support the delivery of anticipatory care plans
• Take responsibility for continuous improvement and quality initiatives within the organisation
• Accept delegated responsibility for a specific area (or areas) or the QOF
• Attend and contribute effectively to external and internal meetings as required
• Contribute effectively to the development and maintenance of the organisation including clinical governance, training, financial management and HR

• On a rotational basis, undertake telephone triage and duty doctor roles

• Identify, implement and embed best practice through clinical guidelines and the audit process
• Contribute to the training of medical students from all clinical disciplines

• Remain fully focused on the strategic aims of the organisation, making recommendations to enhance income and reduce expenditure
• Prepare and attend partnership meetings contributing in a positive manner

• Participate in the management of patient complaints when requested to do so, and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events

• Attend a formal appraisal at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

Experience

• A good understanding of the primary care contract
• Good understanding and knowledge of QOF and of enhanced services
• Proven ability to evaluate the safety and effectiveness of own clinical practice
• Experience of medicines management

• An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners

Clinical Knowledge & Skills
