Overview
Care Navigator, Leeds — full-time. NHS Pension, 5+ weeks annual leave. A permanent Care Navigator position available immediately, with the practice happy to wait for the right candidate. You will join a supportive team within a large GP training practice serving around 19,000 patients in North Leeds. The surgery has a multidisciplinary team including GPs, nurses, HCAs and support staff, creating a busy but collaborative environment where teamwork is key.
Responsibilities
- Answer and manage incoming phone calls; support patients with enquiries; help them access the right service or team member.
- Provide reception cover when needed; greet patients in person and ensure they are supported in a calm, professional, and efficient way.
- Help patients navigate their healthcare journey; listen, understand, and guide them to the right care quickly and safely.
- Empower patients to access the right clinician at the right time; help the whole practice run smoothly behind the scenes.
