Charlton Hill Surgery has a fantastic opportunity for an experienced respiratory nurse to join our dedicated practice nursing team. You will be passionate about getting results and making a difference to the lives of our patients with respiratory issues and we are seeking someone who can provide care, education and support to this group of patients.

We are looking for someone to work 30 hours Monday to Thursday, although we will consider three of these days.

Main duties of the job
  • You will be the lead respiratory nurse looking after our COPD and asthmatic patients, alongside your colleagues.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Be up to date with all necessary training in this field.
  • In addition you will take on a full range of practice nursing duties, including but not limited to immunisations (adult and child); cytology; QOF reviews; and travel health.
  • Ensure QOF is up to date for all patients.

Job responsibilities
  • Manage the nursing care for individuals and groups within the practice population, including direct access to specialist nursing care for patients within the practice setting. Liaise as appropriate with specialist nurses.
  • Undertake diagnostic, health surveillance, health screening, and therapeutic interventions within a broader health promotion/public health strategy which includes responsibility for health promotion information in the practice.
  • Manage programmes of care for patients with acute and chronic disease by planning, providing and evaluating care including audit, recall and evidence based practice.
  • Work with and participate in the development of protocols, policies and standards as agreed within the practice.
  • Contribute to the identification of priorities and goals for the nursing team in consultation with other members of the practice team.
  • Participate in prescribing according to agreed patient group directives (PGDs).
  • Provide nursing expertise to the practice team acting as a source of expert advice on nursing and health issues.
  • Other duties as required by Partners/ GPs & Practice Manager.
