We are looking for an Adult\βs Social Worker to join a Learning Disabilities Reviews Team.
This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About the team
This team undertakes high quality Care Act compliant reviews and re-assessments. A key aspect of this position is to use appropriate risk management and an MDT approach when dealing with each individual service user. Advocating for service user independence when appropriate will be imperative β especially when presenting care needs at commissioning panels.
About you
The successful candidate will be a professional well versed in completing personalised care plans and re-assessing pre-existing plans in a timely manner. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience working in an Adult\βs Social Work setting are essential for this position. A valid UK driving licence and vehicle is essential for this role.
