We are looking for an Adult's Social Worker to join a Community Hospital Team. Do not apply for this job role if you do not have 2 years' permanent experience post qualification.
This team works with individuals aged 18 in hospital settings ready to be discharged. A key responsibility is to ensure safe, person-centred discharge from hospital by assessing needs and arranging the right support to promote independence and wellbeing. The team will be required to carry out face-to-face assessments, Care Act 2014 and MCA assessments.
About you: It is essential to have a degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience within adult's Social Work post qualified. Experience working in a hospital, CHC or short term assessment team lends well to the requirements of the role. A valid UK driving license and regular access to a vehicle is essential in order to be considered for this role.
