We are currently looking for an Adult's Social Worker to join a Reviews Team. This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About the team:
- The team deals with vulnerable adults in the community to review existing care plans, packages and placements to ensure the right level of support and funding is given.
- Responsibilities include assessing service users under the Care Act 2014 as well as bringing cases to commissioning panel when necessary.
- The team is highly proactive and ambitious to ensure all packages are reviewed and adjusted within a reasonable timescale.
About you:
- A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years of experience working in Adult's community based teams is essential for this position.
- The successful candidate will have recent and comprehensive experience completing learning disability placement reviews.
