We are looking for a Social Worker to join a a secure mental health inpatient unit for older adults (male and female).
This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.
About the team
- Attend regular ward rounds as part of the multidisciplinary team, contributing to patient reviews and discharge planningCoordinate safe, timely discharges for patients once they are mentally well - sourcing the right onward setting, whether that's a care home, residential placement, or a return homeExtensive joint working with Adult Social Care, including requesting and coordinating Care Act assessments to support discharge planningWork with patients subject to Ministry of Justice restrictions under Sections 37/41 of the Mental Health Act, navigating the associated legal frameworkManage Section 17 leave applications and discharge processes as part of the patient pathwayA role suited to a social worker confident in mental health legislation, multi-agency coordination, and person-centred discharge planning
About you
A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience is essential in order to be considered for this role. Experience in a mental health setting and working with older adults.
