This is an opportunity to join a respected community-based organisation supporting people experiencing homelessness, poverty, addiction and mental health challenges. The organisation provides practical frontline support alongside longer-term interventions that help people access housing, healthcare and life-changing services.
About the Role
This is a highly rewarding frontline position supporting individuals who may be experiencing homelessness, rough sleeping, housing insecurity, substance misuse or mental health challenges. Working as part of a small, dedicated team, you will provide assessment, triage and casework support, helping people access the services and resources they need to improve their circumstances and wellbeing.
Key Responsibilities
- Conduct assessments and support planning with service users
- Provide person-centred casework and advocacy support
- Connect individuals with healthcare, housing and specialist services
- Support homelessness prevention initiatives
- Maintain accurate case records and outcome tracking
- Build effective partnerships with external agencies
- Contribute to safeguarding and trauma-informed practice
- Work collaboratively with volunteers and colleagues
About You
We are interested in hearing from people who have:
- Experience supporting vulnerable adults
- Experience within homelessness, housing, community health, outreach, substance misuse or related services
- Strong assessment and case management skills
- Excellent communication and relationship-building abilities
- Experience using case management or CRM systems
- Knowledge of safeguarding and trauma-informed approaches
- The ability to work independently and flexibly within a fast-paced environment
