Position: RN Day Triage
The RN Day Triage provides telephonic and ad hoc support to patients and families. The RN, Day Triage will establish and maintain positive relationships with team members, patients/families, and referral resources.
Qualifications:
- Current Affiliate state or multi-state license to practice as an RN, with the ability to apply for/obtain multi-state RN license within 30 days of hire, if not already held.
- Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
- Experience working with an EMR/EHR system.
- Effective communication skills in English; bilingual in Spanish preferred.
- Strong critical decision-making, clinical, and assessment skills.
- Ability to use equipment with visual and auditory mechanisms.
- Excellent time management and ability to prioritize demands.
- Professional handling of stressful situations with exceptional customer service.
- Strong patient service skills, including probing, accurate knowledge transfer, and listening skills.
- Ability to navigate in a fast-paced environment and work cooperatively in a team.
- Sense of urgency and proficiency in computer operations, MS Office, online research, data entry, and navigation.
- Availability to work shifts accommodating extended hours.
- Ability to perform sedentary work with frequent interruptions and manage emotional stress related to terminally ill patients.
- Physical ability to lift patients/equipment, bend, push/pull, kneel, with or without accommodations.

- Triages phone calls efficiently with excellent customer service.
- Provides direct care to patients and families, incorporating psychosocial, spiritual, cultural, physical, and biological components.
- Handles clinical inquiries, promotes services, and documents interactions accurately.
- Coordinates in-person visits when needed, reassures patients and families, and resolves complex situations using appropriate resources.
- Documents patient information and communicates with the interdisciplinary team.
- Reports technical issues promptly and follows emergency protocols during disasters.
- Performs other duties as assigned.
