The Channel Community Care Coordinator is responsible for supporting individuals within the community by delivering high-quality, person-centred care services. The role involves coordinating care plans, liaising with families and external professionals, and ensuring that all services are delivered in line with regulatory standards and organisational values.
Key Responsibilities
- Deliver and coordinate person-centred care that promotes independence, dignity, and wellbeing.
- Act as a key point of contact for service users, families, and relevant stakeholders.

- Ensure compliance with safeguarding policies, health and safety regulations, and care standards.
