The Channel Community Care Coordinator is responsible for supporting individuals within the community by delivering high-quality, person-centred care services. The role involves coordinating care plans, liaising with families and external professionals, and ensuring that all services are delivered in line with regulatory standards and organisational values.

Key Responsibilities

  • Deliver and coordinate person-centred care that promotes independence, dignity, and wellbeing.
  • Act as a key point of contact for service users, families, and relevant stakeholders.
  • Develop, implement, and regularly review individual care plans tailored to service users’ needs.
  • Ensure compliance with safeguarding policies, health and safety regulations, and care standards.
  • Monitor and assess the quality of care provided, taking corrective action where necessary.
  • Maintain accurate and up-to-date records in line with data protection requirements.
  • Support service users in accessing community resources and activities.
  • Work collaboratively with healthcare professionals, social workers, and partner organisations.
