Change Grow Live is seeking a Foundations of Personal Wellbeing Advisor to deliver a structured set of wellbeing sessions for male service users referred by the National Probation Service.

You will conduct initial assessments, develop action plans, and deliver 11 sessions across Derbyshire with travel to other sites reimbursed.

The role requires strong organisational skills, excellent time management, and the ability to write detailed reports on each service user.
